Any of these patterns warrants blood testing including thyroid function, fasting glucose and insulin, full blood count, and depending on other symptoms, hormonal panels for PCOS assessment.
Unintentional significant weight loss — losing 5 percent or more of body weight over six to twelve months without trying — is always worth investigating. While it may reflect manageable causes including increased stress, reduced appetite during a difficult period, or a medication effect, unexplained significant weight loss should never be attributed to positive lifestyle changes without confirmation that it is actually explained.

Weight changes during the teenage years require a specific and careful approach because several overlapping factors are present simultaneously.
Normal puberty produces significant and expected weight change. The adolescent female body is designed to gain a meaningful amount of weight during puberty — including an increase in body fat percentage that represents healthy hormonal function and reproductive readiness. This puberty-related weight change is not excess weight and should not be framed as a problem requiring correction.
Alongside normal pubertal change, teenage girls are at significant risk of disordered eating — whether clinical eating disorders or subclinical restrictive or compensatory behaviours. Unexplained weight loss in a teenager requires careful assessment that considers both medical causes and disordered eating, as the latter may be present even when denied.

Weight gain during puberty is normal, expected, and physiologically appropriate. The body is growing, body fat percentage is increasing to support hormonal function, and the changes are driven by healthy developmental processes. Well-intentioned but misplaced concern about puberty-related weight gain can create the foundation for a disordered relationship with food and body that persists into adulthood.
Weight changes during the reproductive years most commonly reflect the hormonal, lifestyle, and metabolic factors described throughout this guide. Track body weight at the same time of day (morning after using the bathroom, before eating and drinking) on the same day each week — not daily, since daily fluctuation is noise rather than meaningful signal. The weekly pattern over a month reveals whether weight is stable, trending upward, or trending downward.
Track your menstrual cycle alongside weight, since the premenstrual fluid retention pattern is most visible when the two are compared simultaneously.
Track symptoms alongside weight changes — any new fatigue, changes in hair, changes in bowel habits, mood, sleep, or appetite — since these associated features are often the most diagnostically informative element of an assessment.
Track medications and supplements, since new medications can be a direct cause of weight change.
Note whether dietary or activity patterns have changed, even subtly — this is important context for distinguishing lifestyle-driven from medically driven weight change.
A dietary pattern built around vegetables, whole grains, legumes, quality protein, and healthy fats provides the nutritional foundation for weight stability. It supports stable blood sugar, reduces insulin spikes that promote fat storage, provides adequate satiety per calorie, and meets the micronutrient needs that support hormonal and metabolic health. Consistency across most days — not perfection on any specific day — is what produces stability over months and years.
The combination of regular aerobic activity (at least 150 minutes of moderate activity per week) and strength training (two to three sessions per week) supports healthy body composition more effectively than cardiovascular exercise alone.
